Learning Objectives and Outcomes
I. Confined space
- Limited or restricted entry or exit
- Large enough for employee to enter and perform work
- Not designed for continuous occupancy
II. Permit-required confined space
- Contains or could contain a hazardous atmosphere
- Contains a material that has the potential for engulfing an entrant
- Has internal configuration that might cause an entrant to be trapped or asphyxiated
- Contains any other recognized serious safety or health hazards
III. Requirements for a written program
- Identify and evaluate hazards before entry
- Test and monitor conditions during entry
- Implement necessary measures to prevent unauthorized entry
- Establish the means, procedures and practices to eliminate or control hazards necessary for safe permit-space entry
- Identify job duties
- Provide, maintain and require all necessary PPE’s
- Ensure at least one outside attendant during entry
- Implement procedures for summoning rescue and emergency services
IV. Entrant’s training and duties
- Know space hazards
- Use appropriate PPE’s
- Maintain communication with attendants to enable attendant to monitor status
- Exit ASAP when ordered to by authorized personnel or when entrant recognizes warning signs or symptoms of exposure, or when alarm is sounded
- Alert attendant when prohibited conditions or warning signs exist
V. Attendant’s duties
- Remain outside unless relieved by another authorized attendant
- Perform non-entry rescues when specified
- Know existing and potential hazards
- Maintain communication with and keep an accurate account of workers entering the permit-required space
- Order evacuation when a prohibited condition, when a worker shows signs of physiological effects of hazard exposure, when an outside emergency exists and when the attendant cannot perform required duties
- Ensure that unauthorized persons stay away or exit immediately
- Inform entrants and entry supervisor of entry by unauthorized person
- Perform no other duties that might interfere with attendant’s primary duties
VI. Entry Supervisor’s Duties
- Know space hazards
- Verify emergency plans and specified entry conditions before allowing entry
- Terminate entry and cancel permits when entry operations are completed or if a new condition exists
- Verify that rescue services are available and that the means for summoning them are operable
- Take appropriate measures to remove unauthorized entrants
- Ensure that entry operations remain consistent with the entry permit and that acceptable entry conditions are maintained
